The Role

We are seeking a compassionate and experienced Health Coach who understands that caring for people with chronic conditions is more than a job—it's a calling. You will be a force of positive change for individuals managing complex health conditions, speaking with both compassion and authenticity while fostering faith in what's possible with proper support and resources.

You operate from deep empathy, asking tailored questions and working collaboratively with members to identify solutions that fit their unique circumstances. Members will view you as a trusted confidante, knowing you listen without judgment and will go the extra mile to help them thrive.

Key Responsibilities

• Deliver motivational support and health education to 9amHealth members via Zoom for conditions including obesity, diabetes, hypertension, high cholesterol, and weight management
• Apply motivational interviewing and evidence-based coaching techniques to facilitate lasting behavioral changes in nutrition, sleep hygiene, physical activity, and stress management
• Maintain organized scheduling calendars to support seamless member visit coordination
• Personalize coaching sessions using member health data to support individualized goal achievement
• Document all member interactions accurately in our electronic medical record (EMR) system
• Reinforce nutrition and lifestyle plans developed by registered dietitians, tailoring recommendations to each member's unique goals and preferences
• Escalate member concerns appropriately and contribute to delivering exceptional care experiences

Required Qualifications

• Active NBC-HWC (National Board Certified Health and Wellness Coach) certification
• Prior professional experience working within a health and wellness organization
• Demonstrated expertise in supporting individuals managing chronic conditions
• Strong proficiency with telehealth platforms and digital health tools
• Excellent written and verbal communication skills
• Ability to document accurately in electronic medical records
