  • Develop individualized medical nutrition therapy plans addressing chronic conditions, dysphagia, renal disease, diabetes, and other specialized dietary needs
  • Collaborate with interdisciplinary care teams including physicians, nurses, and care coordinators via telehealth platforms
  • Monitor resident progress and adjust nutrition interventions based on clinical outcomes and lab values
  • Ensure compliance with state and federal regulations governing long-term care nutrition standards (including CMS guidelines)
  • Maintain detailed and accurate electronic health records (EHR) documentation
  • Provide staff education on nutrition protocols, specialized feeding techniques, and dietary accommodations
  • Participate in care planning meetings and quality improvement initiatives
  • Review medication-nutrient interactions and provide recommendations to clinical teams
  • Support residents and families with nutrition counseling and dietary education

Required Qualifications

  • Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential via the Commission on Dietetic Registration (CDR)
  • Valid license to practice as a Registered Dietitian in the United States (or eligible for licensure by state of employment)
  • Minimum 2+ years of clinical dietetics experience, preferably in long-term care, skilled nursing, or geriatric nutrition
  • Proficiency with electronic health records (EHR) systems and virtual care platforms
  • Strong knowledge of medical nutrition therapy and therapeutic diets
  • Excellent communication skills and ability to work effectively in virtual team environments
  • Understanding of regulatory requirements in long-term care settings
  • Reliable high-speed internet connection and professional home office setup

Preferred Qualifications

  • Specialist Certification in Gerontological Nutrition (CSG) or Renal Nutrition (CSR)
  • Experience with telehealth platforms and virtual patient care delivery
  • Knowledge of quality metrics and outcomes measurement in long-term care
  • Background in nutrition support and enteral/parenteral nutrition
